“Health in your hand”- Promotion of HA Go App “DM care” utilization in DM education

Introduction :
Since HA Go and its features of recent enhancement of “DM care" app can assist DM patients in the management of their own health more lively and effectively. HA Central Committee on Diabetic Service (CCDS) encouraged DM nurses to promulgate these new features to our patients. According to the statistic in SOPCs, FMSCs and GOPCs 2013-2023, the number of DM patients in HA had reached 56,9400 in Year 20231. Yet, the total number of patients prescribed with HA Go prescription was 9,594 only 2. Diabetes and endocrine center is indeed the optimal strategic platform for promoting and integrating the DM Care App into patients’ care journey.
Objectives :
Through the program
- Manage health care at patient’s +/- carer’s fingertips more effectively
- Increase patient awareness of DM self-care management
- Empower patient to manage their own health
- Facilitate carer(s) to manage patient health
- Promote green living by using Telehealth to prescribe educational materials / videos
Methodology :
- Use DM Care Dummy account for clinical staffs’ training by HA Go (AAT) DM Care demo environment
- DM patients who are referred to out-patient DM education including OPAS code (DMNE and DAIT) during 1.1.2025 to 11.3.2025 (10 weeks) were recruited as target
- Introduce benefits of HA Go and “DM Care” to patients +/- carers - Encourage patients +/- carers to register HA Go app as “Full member” or “Designated Carer” of patient
- Assist in installation of HA Go app if needed
- Satisfactory survey for the functions of HA Go App
Result & Outcome :
This CQI project successfully promoted the adoption of HA Go app within a cohort of 698 diabetes patients. The interventions achieved a significant increase in app penetration, from 54.0% to 81.4%. Post-implementation evaluation revealed exceptionally high user satisfaction, with over 90% of respondents find the app convenient and helpful for managing their diabetes. The appointment and blood glucose recording are highly utilized among the functions of HA Go App. Moreover, the result demonstrated a strong patient preference (73.8%) for receiving educational materials digitally via the app. Key barriers to adopt the app were identified, including lack of digital literacy, negative perceptions of app utility and technical issues. The project had successfully shifted the service model towards a new era of digital-first approach, while also emphasizing the ongoing requirement for inclusive and alternative options for minority of patients.
