Introduction :
The transition from hospital to home is a vulnerable period for stroke patients and their families. This journey is often accompanied by anxiety about managing new health needs. A nurse-led initiative was launched in August 2025, to provide support during this critical period.
Objectives :
1. To connect patients and family carers to the Patient Resource Centre’s Pre-Discharge information session.
2. To deliver and evaluate a hybrid multidisciplinary empowerment program at the Geriatric Day Hospital (GDH) for its impact on confidence and caregiver burden.
Methodology :
The initiative was conducted in two phases
Phase 1 (In-patient Preparation): Nurse invited stroke patients and carers to the Patient Resource Centre’s Community Resource Talk.
Phase 2 (GDH Empowerment): We delivered a hybrid program. It included educational videos from our team: Physiotherapists and Occupational Therapist on daily exercise; Nurse on stroke care; Doctor on stroke and its prevention; and Medical Social Worker on community resources. This was followed by a live, Question and Answer (Q&A) session. Participants joined in- person or remotely via the HA GO. Impact was measured with a post- session survey.
Result & Outcome :
From August to December 2025, a total of fitty participants (30 patients, 20 carers) attended the GDH Stroke Patient and Carer Empowerment Program, with 11 families participated remotely via HA GO. Feedback was overwhelmingly positive, with a 98% response rate (49/50). All respondents reported satisfaction, and 45–49% rated the program content and delivery as “very satisfied.” Importantly, 100% strongly supported continuation of the program. Participants highlighted that the practical guidance improved daily living, boosting confidence, and reduced anxiety for both patients and carers.
Conclusion:
This nurse-led initiative demonstrates how structured, team-based education and early linkage to community resources can transform the stroke recovery journey. By bridging hospital discharge with accessible empowerment programs, nurses help families move from worry to confidence, fostering resilience and strength in recovery.
