Strengthening Foley Catheter Fixation and Care Practices Among Nurses and Supporting Staff

Introduction :
Foley catheters are commonly used medical devices for patients requiring urine drainage. Despite their routine use, improper catheter care—particularly inadequate fixation—can result in discomfort, urinary tract infection, or urethral trauma. To enhance staff knowledge and practices related to Foley catheter fixation and care procedures, two identical workshops were organized, aiming to improve patient safety and restore high standards of care.
Objectives :
1. To reinforce correct Foley catheter fixation techniques and emphasize the importance of proper adherence 2. To enhance overall staff competency in catheter care, including emptying the bedside urine bag, performing perineal care, and preventing paraphimosis 3. To improve staff compliance to Foley catheter care and fixation
Methodology :
Two identical workshops were conducted using a structured educational program for patient care assistants (PCA) and nursing staff. The training consisted of a pre and post-training quizzes, theoretical sessions on catheter structure, function, and care, live demonstrations, and hands-on simulation practice. Four key areas were covered: (1) correct technique for emptying urine bags, (2) proper catheter fixation using appropriate techniques, materials, and fixation sites, (3) perineal care, and (4) prevention of paraphimosis through proper foreskin repositioning. A post training quiz and direct observation with feedback during return demonstrations and simulations were used to assess knowledge improvement and hands-on performance. Compliance audits were planned to monitor compliance with Foley catheter care standards.
Result & Outcome :
A total of 70 staff participated in the workshops on 11 December 2025 (47 nurses, 15 PCAs, and 8 nursing students). The mean quiz scores improved from 75/100 before training to 81.3/100 after training, demonstrating enhanced understanding of fixation principles and catheter care workflows. Participants demonstrated correct fixation techniques and increased awareness of the risks associated with improper fixation. Staff confidence in catheter care practice improved noticeably, and demonstrated greater initiative in monitoring fixation, assessing skin integrity, and performing Foley catheter care during daily routines. A compliance audit is planned to evaluate the sustainability of improvement and consistency in catheter fixation and compliance.
