Introduction :
Safe and effective discharge is a critical component of patient care continuity. However, patient feedback collected through the Patient Feedback Form indicated that discharge procedures were often perceived as insufficiently explained. Common issues included confusion regarding medication collection, uncertainty about follow-up appointments, and anxiety related to wound care. Discharge education was largely dependent on verbal explanations provided at a short time, increasing the risk of information being misunderstood. Therefore, a standardized Patient Discharge Checklist integrated with QR code–linked educational resources, including animation videos, was developed and implemented in the Orthopaedics & Traumatology Department.
Objectives :
To improve patient understanding of discharge procedures To address issues of “insufficient discharge information” highlighted in Patient Feedback Forms To standardize discharge education among nursing staff To reduce repeated patient enquiries related to medications, follow-up, and home care To enhance patient experience through accessible digital education tools
Methodology :
A structured Patient Discharge Checklist was developed covering key domains: Medications Wound and home care Follow-up arrangements Red flag symptoms Clear instructions were included on: Where to collect discharge medications Payment procedures Medical record applications QR codes were embedded in the checklist, linking to: Medication safety education Wound care. Cast and splintage care. HA Go app and community support services. Short animation videos were used to visually explain discharge instructions in a patient-friendly format. Nurses reviewed the checklist step-by-step with patients and caregivers. The checklist was provided for home reference.
