Introduction :
This retrospective case series assessed a biodegradable temporizing matrix (BTM) in 8 complex orthopedic wounds. All achieved successful staged closure, with a mean 25-day vascularization time and minimal complications (one partial graft loss, one infection). The study concludes BTM is a reliable, flap-sparing reconstruction option.
Objectives :
This study assessed the Biodegradable Temporizing Matrix (BTM) for staged reconstruction of complex orthopedic wounds with exposed bone or hardware.
Methodology :
A retrospective series of eight patients followed a protocol of radical debridement, BTM application with initial negative pressure therapy, and delayed management after neodermis formation (~4 weeks) via split-thickness skin grafting or secondary intention healing. Successful closure at three months was the primary outcome.
Result & Outcome :
All eight wounds achieved successful closure. 50% (4/8) required only skin grafting over BTM, avoiding complex flap surgery. Mean BTM vascularization time was 25 days. Complications were minimal (one minor partial graft loss, one superficial infection). No BTM failures occurred, and functional mobility was restored in 75% of cases. BTM is a reliable, effective reconstructive option. It successfully generates a neodermis, reduces reliance on flap procedures, and achieves stable coverage with a low complication profile when meticulous technique is used. Impact For nursing, BTM simplifies postoperative care by stabilizing the wound bed and reducing dressing frequency. The clear visual cue of a vascularized neodermis provides an objective indicator for graft timing, improving team communication. This predictability can streamline recovery, potentially shortening stays and allowing earlier focus on rehabilitation and patient education.
