Introduction :
A comprehensive and integrated care bundle for incontinence-associated dermatitis (IAD) is essential for preventing IAD and sustaining high-quality care delivery in hospital settings. This bundle incorporates evidence-based protocols, thorough staff education, active personnel engagement, meticulous documentation, and rigorous reporting mechanisms. Although the New Territories West Cluster (NTWC) guidelines for IAD prevention and management exist, targeted reinforcement is imperative to maintain elevated care standards
Objectives :
To elevate standards of care for IAD prevention and management at Pok Oi Hospital (POH). To augment the knowledge and competencies of nursing and support staff in IAD prevention and management. To strengthen frontline personnel's understanding of the cluster-wide IAD prevention and management guidelines.
Methodology :
Following a panel discussion involving managers and wound link nurses, a one month comprehensive educational program was implemented across the inpatient care units of Pok Oi Hospital (POH). A multifaceted intervention strategy was adopted, incorporating the introduction of a streamlined skincare protocol to address identified gaps in clinical practice. The protocol specified the application of Vaseline for skin protection in high risk or Category 1 incontinence-associated dermatitis (IAD) cases; the use of Coloplast Critic Barrier Cream for Category 2 lesions involving skin loss; and the application of supplementary paraffin gauze to peri wound areas. Furthermore, the standardized Turning and Skincare Chart in the Medicine and Geriatrics (M&G) Department at POH was revised, alongside modifications to the Intake and Output (I&O) Chart in the Mixed Specialty Wards (MSW) at POH. A variety of educational modalities—including roadshows and hands on practical sessions—were delivered to frontline nursing and supporting staff to facilitate protocol implementation and reinforce best practices.
Result & Outcome :
Implementation of the simplified skincare program resulted in an approximate 50% reduction in IAD-related wound consultations. Moreover, affected skin areas demonstrated decreased severity and improved healing potential. These findings highlight the effectiveness of the educational program in enhancing the quality of care delivered through the IAD care bundle. Comprehensive and integrated IAD care bundles have been shown to be effective in both the prevention and management of incontinence-associated dermatitis, thereby maintaining skin integrity and mitigating the risk of progression to pressure injuries.
