Introduction :
To strengthen telehealth support for elderly homes (RCHEs), enhance service effectiveness, and facilitate access for RCHEs, patients, and Community Geriatric Assessment Team (CGAT) staff, the Hospital Authority (HA) Cluster Performance Department has initiated the development of institutional telehealth. Objectives :
Institutional Telehealth is a one-stop service platform that facilitates patients within institutions to receive healthcare services through streamline digital communication technologies.
Methodology :
Methodology: Before consultation: The process included Create, Pending and Returned before the consultation. CGAT Staff create consultation list or receive consultation list submitted by RCHE then view consultation list and update consultation list. On consultation day The process of confirmed by CGAT Staff once the list was finalized. When ready for consultation, CGAT doctor scan QR code and to start the zoom consultation with RCHE. After consultation The process of completed the list by CGAT staff right after consultation.
Result & Outcome :
