Introduction :
It is a prevalent scenario in Hong Kong that patients are unable to identify their latest medications at home during drug counseling or adherence assessment. Pharmacists often describe the physical characteristics of the medication such as its shape and color to trace the latest regimen. However, it could be time consuming and some medications may share similar appearance. The Medication Image Library mobile app has been overcoming these difficulties by providing a convenient way to display medication images to patients. At TWH, clinical ward pharmacy services include drug adherence checks and discharge counseling. The Medication Image Mobile App will aid pharmacists by providing a reliable platform to display medication images, enhancing patient understanding, improving adherence, and ensuring safer discharge counseling, thus enhancing pharmaceutical care quality.
Objectives :
To develop a portable reference tool for displaying the appearance of medications in Tung Wah Hospital (TWH) for assisting both patients and healthcare professionals to identify medications. To Facilitate the followings service: - Medication Reconciliation - Medication Compliance checking - Medication counselling
Methodology :
1) To regularly update the Medication Image Library Mobile App to display both existing and the past medication image.
Result & Outcome :
To be utilized by healthcare professionals across HKWC and potentially the entire Hospital Authority. 1) Utilization rate of healthcare professionals. 2) The accuracy of the drug image database
