Colorectal Cancer Note
Introduction: :
The Cancer Case Manager (CCM) Programme was introduced by the Hospital Authority (HA) to enhance care coordination and continuity for cancer patients. Cancer Note is a key electronic platform supporting CCM workflow, clinical documentation, performance monitoring, and service evaluation. Head Office review of 2023/24 Cancer Note data identified suboptimal documentation compliance in the Colorectal CCM Programme, including incomplete multidisciplinary team (MDT) records, missing reasons for non-MDT cases, delayed programme exit. These gaps may compromise data accuracy and programme evaluation.
Objectives: :
This CQI project is improve the compliance, accuracy, and consistency of Colorectal Cancer Note documentation among Cancer Case Managers in the Kowloon West Cluster.
Methodology: :
A two-phase CQI intervention was implemented. Phase 1 comprised a structured hands-on educational session with live demonstrations of Cancer Note entry, supplemented by pre-and post-intervention knowledge assessments. Phase 2 involved follow-up friendly visits conducted one month later, including review of Cancer Note entries for newly recruited colorectal cancer patients, scenario-based question assessments, and repeat knowledge testing to evaluate knowledge retention and application in clinical practice.
Result & Outcome: :
Eight Cancer Case Managers participated in the educational intervention. Mean knowledge scores improved from 57% at baseline to 97% immediately post-intervention and further increased to 100% at one-month reassessment, demonstrating sustained knowledge retention. Review of 12 Cancer Notes and three scenario-based question assessments across three hospitals in KWC (PMH, CMC, and YCH) demonstrated 100% compliance, with complete documentation of MDT decisions and reasons, accurate recording of KPI variances, and timely exit from the CCM programme. The CQI project significantly enhanced documentation compliance, accuracy, and standardization in the Colorectal CCM Programme. Structured education combined with follow-up review proved effective in improving both knowledge and practice.
