Introduction: :
Lung cancer was the most common cancer in Hong Kong in 2023, with 6,111 new cases representing 16.1% of all diagnoses. Diagnostic delays contribute to late-stage presentation and poorer survival outcomes. To reduce fragmented care and improve the patient journey, Princess Margaret Hospital established a One-Stop Lung Diagnostic Clinic in August 2021 and aligned with the Hong Kong Cancer Strategy 2019. Building on these initiatives, the Cluster Lung multidisciplinary team (MDT) proposed a Lung Cancer Case Manager (CCM) in October 2025 in KWC to streamline care, coordinate interdisciplinary input, and enhance patient education and psychosocial support.
Objectives: :
- To improve coordination, efficiency, safety, and patient outcomes. - To reduce waiting times throughout the diagnostic and treatment pathway. - To strengthen multidisciplinary teamwork and enhance patient centered support. - To establish the sustainability and scalability of the Lung Cancer Case Manager role as a model for the Hospital Authority.
Methodology: :
The lung cancer case manager's interventions within MDTs enhance care coordination in line with the HA CCM Operational Manual. The CCM provides coordinated support from diagnosis through follow-up. After initial diagnosis of lung cancer, they coordinate investigations and multidisciplinary team activities, streamline appointments across different disciplines, and provide psychological support and education to facilitate decision-making. In follow-up, they monitor treatment toxicities, ensure adherence to follow-up plans and initiates review with medical team according to patients' needs, and coordinate surveillance.
Result & Outcome: :
Under the lung cancer CCM pathway, CCM manages about 180 new cases per year, providing each patient with at least four face-to-face visits and telecare contacts. The pioneering lung cancer case manager program enhanced patient care and operational efficiency by aligning multidisciplinary professionals through dedicated case management. This innovative service model delivers the competent, personalized care outlined in the HA Strategic Plan 2022–2027, which aims to improve patient outcomes and operational efficiency.
