Introduction: :
Patient falls are a significant concern, particularly among ophthalmic patients with visual impairments. Since 2024, there have been 11 reported cases of falls among ophthalmic patients within the hospital, in which 6 cases happened before follow-up, 3 cases happened during follow-up and 2 cases happened after follow-up. To address this issue, the Eye OPD Working Group has been established to implement multiple fall prevention strategies.
Objectives: :
To reduce the fall rate among ophthalmic patients by implementing effective fall prevention strategies.
Methodology: :
Proactive measures to empower all ophthalmic patients: - Training patient safety ambassadors to engage with patients - Distributing easy-to-read leaflets on fall prevention in waiting hall - Broadcasting QMS announcements on fall prevention - Showing videos on fall prevention tips at G/F lift lobby - Pre-printing QR code linking to fall prevention tips on appointment slips - Offering way guidance assistance to pharmacy or exit.
Result & Outcome: :
- Fall Incidents: 3 fall cases were reported post-implementation of the preventive strategies in which all cases happened before follow-up and occured outside the hospital premises. - Surveys: A total of 30 patients and 20 staff members were interviewed. Patient fall prevention awareness scores averaged 3.91, while staff satisfaction scores averaged 4.6 following the implementation of fall prevention strategies. - Patient Safety: Enhanced patient safety during and after consultation. - Patient Empowerment: A significant improvement in patient empowerment regarding fall prevention. - Future Goal: Aim to establish a barrier-free pathway towards Eye OPD by the collaboration of Hospital Administrative Department and District Council member, to provide a safe and accessible route for patients.
