Introduction: :
This CQI project aims to establish a Telehealth Nurse clinic dedicated to providing pre operative education for patients after neo-adjuvant therapy (chemotherapy). By leveraging technology, we can empower patients and their caregivers, promote collaboration among healthcare providers, and drive innovation in nursing care to improve in patient outcomes.
Objectives: :
1.Empower Patients: Provide accessible pre-operative education to enhance patient understanding and engagement. 2.Facilitate Collaboration: Enable caregivers to participate in consultations, regardless of their locations. 3.Enhance Accessibility: Reduce travel time and associated stress for patients and their families 4.Improve Outcomes: Monitor effectiveness of the telehealth intervention by post operation drain care technique and experience surveys satisfaction as outcome measures.
Methodology: :
1. Patient recruitment: Collaborate with surgeon and Breast cancer case manager to identify eligible patients. Seamless from hospital FU to home care empowerment kit: video QR code, post op education pamphlet, drain bottle dummies, enquiry hotline and necessaries. 2. Technology setup: Use HA Go as a telehealth platform that support most of the smart phone for video consultation and sharing information. Assist patient download the latest version of HA Go and set up the account for both patients and carers. Educate patient to enter the consultation by notification and telehealth icon Electronic Payment options is enabled either from patients or carers. Ensure secure and compliant data handling.
3. Pilot program design: Proposed in KEC nurse clinic workgroup for endorsement 5/2024 Launch a pilot program with target group of patients 12/2025 Recruit experience clinic nurse trained in telehealth 12/2025 4. Evaluation and feedback: Use Google form for collection patient feedback after telehealth consultation. Review outcome regularly to identify areas for improvement.
Result & Outcome: :
Increase patient knowledge regarding surgical procedures and post operative care, patients and relative able to recall the procedure of handling the wound drain. Enhanced satisfaction (87.5% strongly agree and 12.5% agree) reported by both patients and caregivers. Reduction in travel related stress and time for patients and families. Establishment of a scalable telehealth model for ongoing pre-operative education.
