Introduction: :
Yan Chai Hospital provides total knee replacement (TKR) for patients regardless of district. Some post TKR patients living distant face challenges in visiting our outpatient clinic. Telehealth serves a convenient option for patients with limited mobility. Besides, current outpatient setting has limitation in tracking patients’ home exercise performance. Telehealth overcome this limitation by offering real-time feedback and supervision. It ensures patients exercise correctly and encourages frequent completion of home exercise program. Hence, we introduced a dual mode rehab pathway “FATE” for them:
FA- face-to-face session, to assess patients and prescribe individualized exercises in-person
TE – telehealth session, for re-assessment and tracking home exercise progress. After attending first “FA” session, patients will be arranged both “FA” and “TE” sessions throughout the course of treatment. For patients with stable condition, they will be reassessed via “TE” session. Home exercises will be progressed via HAGo prescription.
Objectives: :
–To promote better home exercise technique by providing guidance throughout the session.
–Utilizing home facilities as equipment for exercise, hence to improve exercise compliance.
–To improve accessibility to physiotherapy service for cross-district patients.
–To provide regular reassessment for stable post-TKR patients via Telehealth.
Methodology: :
–Patients with: 1. HA Go, 2. smartphone, 3. interest in attending TELE-training were recruited into the program. –“TE” session is consisted of:
oPre-class assessment
o1 hour home exercise session
oPost-class Q&A session
oHAGo exercise update and prescription
Result & Outcome: :
All of the participants agrees that participating in telehealth class is able to improve their confidence in completing prescribed exercise correctly, enhance their compliance with the home exercise and improve related knowledge. 85% of the participant reports the service allows them to receive physiotherapy service more conveniently and they having been completing home exercise everyday.
