Introduction: :
According to data in 2018-19, the patient fall cases were 14 and 16 respectively in Yan Chai Hospital (YCH) surgical department which 33% happened after operation. Since 2018, YCH has ranked patient fall as 1st in the Top Risk Matrix. Patient fall can lead to injuries and other medical consequences that may extend hospitalization, follow-up actions and even legal actions. A fall prevention program was established afterward and was given a theme each year. In 2025, the aim was to achieve zero fall cases in Surgery in YCH.
Objectives: :
(1) enhancing knowledge on staffs and self-empowerment of patients through education on fall prevention to minimize risk of fall (2) fostering a caring culture within ward by incorporating inputs from diverse stakeholders (3) constantly hold activities to raise alertness on fall prevention
Methodology: :
The effectiveness of the program was measured by the no. of cases of patient fall, the pre and post test results and satisfactory survey from patients. To achieve the objectives, a systematic flow with creative innovations for prevention of fall education was designed. It involved collaboration of ward staff, Enhanced Recovery After Surgery (ERAS) program clinic nurse, physiotherapists, patients and their caregivers. To respond to the theme of the Zero Fall Campaign, there were "Zero Fall Day", "Zero Fall Week" and "Zero Fall Month" in YCH Surgery. In short, activities of educational talk was held on Zero Fall Day; renewal talk, booth games and banner showdown were held on Zero Fall Week; calender countdown and celebrating ceremony were held on Zero Fall Month. To enhance awareness among patients and ward staff regarding to fall prevention strategies, hardware including pamphlet, tailor-made posters and stickers were developed. Furthermore, fall prevention related educational talks were delivered to staffs with pre-post tests. To facilitate new ideas, the ward representatives had regular meetings and then disseminated messages back to wards.
Result & Outcome: :
Zero fall was achieved in YCH Surgery department in 2025 The correct rate in post-test on fall knowledge on ward staffs was improved, especially on questions related to fall assessment and post fall management with 100% accuracy in some questions Over 87% patients were satisfied on the program according the patient survey in 2025
