Introduction: :
Leg ulcer and Stage 2 or above pressure injuries pose significant risks to patient safety and recovery when dressing care is inappropriate. Despite the increasing of availability of advanced dressing materials, the transition to advanced wound care technology requires a corresponding increase in systematic staff training. Current nursing assessments often lack a holistic approach to address tissue regeneration and psychosocial barriers. This project introduces the TIMERS framework (Tissue, Infection, Moisture, Edge, Repair, and Social Factors) to standardize wound bed preparation and integrate patient-centered care.
Objectives: :
The primary goal is to improve healing outcomes by enhancing nursing staff’s clinical knowledge, standardizing wound care protocols, and ensuring comprehensive psychosocial care for patients with leg ulcers and stage 2 or above pressure injuries.
Methodology: :
This quality improvement proposal will be implemented in a clinical department targeting all nursing staff and a sample of 50 patients with leg ulcer and stage 2 or above pressure injuries within 6 months. The intervention centers on the "TIMERS Bundle", which consists of a quick-reference dressing selection guide, a standardized packing method, mandatory dietitian referrals, and self-care education. Staff will undergo educational training using the framework. Success will be measured through pre and post- test to assess staff and patient knowledge. Additional metrics include weekly audits of staff compliance, clinical photography every Thursday, and consistent documentation of wound dimensions.
Result & Outcome: :
The project anticipates a measurable increase in nursing staff knowledge regarding advanced dressing materials, and a significant improvement in patient comprehensive of self-care education. Expected clinical outcomes include a minimum 10–20% Percentage Area Reduction (PAR) in wound dimensions over 2 weeks The TIMERS bundle provides a systematic framework for the application of advanced dressing materials and holistic patient care. The implementation of this bundle is expected to enhance the quality of nursing practice and reduce patients’ length of stay. This project aims to offer a scalable approach for wide adoption in clinical to standardize wound management and improve clinical outcome.
