Introduction: :
With the advancement in medical and surgical management on congential heart disease (CHD), children born with CHD are now expected to reach adulthood. As various medical problems may develop after cardiac surgeries, life-long medical follow-up are required. Their health care needs may also not be met in paeidatric care settings. It is essential to transfer them to adult health care systems for the continuity of care. A Transition Care Program is conducted for adolescents aged 15-18 years who are with moderate to complex CHD to enhance their knowledge on CHD and self-management skills, so as to prepare them transiting from child-centered to adult-oriented health care system.
Objectives: :
To address the creation of the Transition Care Program based on 4M1E (Man, Machine, Material, Method, Environment) model.
